Job summary

Element Care in Beverly, MA seeks a Physician to deliver comprehensive geriatric primary care for frail older adults within the PACE Interdisciplinary Team and contribute to the Center Leadership Team.

The role combines clinical expertise with ownership of participant outcomes and center results, supporting independence and safe community living while advancing population health and value-based care initiatives.

Qualifications

• Completed medical residency required; Internal Medicine, Family Medicine or Emergency Medicine preferred.
• Board certification or approved board eligibility for recent graduates required.
• Current Massachusetts medical license, Massachusetts controlled-substance and DEA authority; appropriate hospital and long-term-care credentials and privileges; current CPR certification.

Responsibilities

• Provides geriatric primary care for complex illness, frailty, cognition, function, falls and prevention; completes examinations and care plans; emphasizes medication optimization and deprescribing with the team.
• Provides urgent assessment, treatment and follow-up in clinic, home or nursing facility; uses telehealth when needed; arranges transfers as indicated.
• Performs home visits and nursing-home visits; participates in hospital rounding, on-call coverage and follow-up.
• Integrates palliative care through symptom management and advance-care planning; coordinates hospice or specialty consultation.
• Identifies and coordinates behavioral health needs with the team; ensures safety assessment and escalation.
• Delivers culturally responsive education; engages interpreters when needed and supports participant autonomy.
• Participates in Center Leadership Team and interdisciplinary reviews; improves workflows and guides colleagues.
• Contributes to reducing falls and avoidable utilization; applies population health and value-based care principles.
• Fosters respectful, accountable culture; escalates safety or HR concerns when needed.
• Adheres to Element Care pathways and maintains documentation, orders and communication; protects confidentiality.
• Travels locally and performs other duties as required.

Education

Medical residency completed (Internal Medicine, Family Medicine or Emergency Medicine preferred)

Massachusetts medical license

Board certification or board eligibility

Geriatrics or palliative fellowship (preferred)
