Job summary

WelbeHealth, LLC. is hiring Primary Care Physicians to provide life-changing, community-based care to our vulnerable seniors through a value-based PACE model.

You will conduct comprehensive in-person assessments, collaborate with the interdisciplinary team, and focus on holistic patient care. Expect a base salary with quarterly bonuses, relocation assistance, CME reimbursement, and growth opportunities toward medical directorship.

Qualifications

• Residency program accredited by the ACGME.
• Master's or doctorate degree required.
• Board certification preferred.
• Active state physician license without restrictions.
• State CDS certification and DEA registration.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Perform comprehensive in-person assessments with the IDT to create a personalized care plan.
• Coordinate discharge for acute, short-term, and long-term placement.
• No hospital rounds; see an average of 8 patients per day.
