Job summary

Element Care seeks a Physician to provide comprehensive geriatric primary care as part of the PACE Interdisciplinary Team. The role emphasizes participant outcomes, center leadership participation and a culture of safety and compassion.

The Physician collaborates with medical and behavioral health colleagues to optimize medications, manage complex cases and support independence and safe community living for frail older adults.

• Ability to work across interdisciplinary teams and settings.

Responsibilities

• Provides geriatric primary care for complex illness, frailty, cognition and function.
• Uses medication optimization and deprescribing in collaboration with the team.
• Performs home visits, nursing-home visits and hospital rounding.
• Coordinates palliative care, goals-of-care discussions and hospice involvement.
• Identifies and coordinates behavioral health needs with the care team.
