Job summary

Center for Elders Independence seeks a Primary Care Physician to deliver medical care within the PACE framework. You will lead an interdisciplinary team and guide care planning for elderly participants, ensuring coordinated, patient-focused services and strong collaboration with families and caregivers.

Qualifications

• MD with current, unrestricted license to practice in California.
• Minimum of 1 year experience with frail/elderly populations.
• Commitment to working in a PACE interdisciplinary team.
• Board certification in Internal Medicine or Family Practice preferred.
• Geriatrics subspecialty training preferred; BLS and DEA certificates required.

Responsibilities

• Provide comprehensive medical care guided by the PACE benefit.
• Conduct initial, 6-month, annual, and periodic assessments.
• Develop and implement participants' care plans with IDT.
• Review and sign care plans every 6 months.
• Supervise CEI nurse practitioners and physician assistants as appropriate.
• Coordinate with family and support networks; arrange follow-ups.
