Job summary

Element Care is seeking a Physician to join the PACE Interdisciplinary Team in Massachusetts. The leader of the Clinical Team will oversee care for high risk older adults living in the community, focusing on geriatric care management and patient goals.

The role emphasizes collaboration with diverse team members, comprehensive exams, home visits, and care coordination to help participants remain in the community safely. Board certification in Internal Medicine or Family Practice is required.

Qualifications

• Board Certified in Internal Medicine or Family Practice, or eligible for Board Certification.
• Board Certified in Geriatric Medicine preferred.
• Current License to practice medicine in Massachusetts.
• Massachusetts DEA Prescription Privileges; Suboxone Waiver preferred.
• Spanish-speaking skills required or bilingual in Spanish & English desirable.
• Experience with geriatric population; at least 1 year.
• Maintains hospital and long-term care credentials and privileges.
• Current CPR certification.

Responsibilities

• Completes initial, periodic and annual physical examinations of participants.
• Evaluates participants for episodic visits, acute illnesses and follow-ups of hospitalizations in clinic, at home or in nursing homes.
• Makes nursing home visits in accordance with state requirements and as needed.
• Uses telehealth as needed.
• Participates in the PACE hospital rounding system and on-call schedule.
• Develops treatment plans in concert with Interdisciplinary Team.
• Provides health care and culturally competent teaching to patients and families.
• Completes documentation in a timely, accurate manner.
• Ensures interactions adhere to high standards of care.
• Passes an N95 mask fit test; adheres to respirator seal requirements.
• Frequent local travel.
• Performs other duties as required.
