Job summary

Center for Elders Independence in California is seeking a Primary Care Physician to oversee medical care for participants in our PACE program. Under the Chief Medical Officer, you will lead the medical team and ensure integrated, high-quality care.

The PCP will perform assessments, guide care plans with the IDT, coordinate with specialists, and provide after-hours coverage as part of a rotating schedule.

Qualifications

• MD with current, unrestricted CA medical license.
• Minimum 1 year of experience with frail or elderly populations.
• Formal training in Internal Medicine or Family Practice.
• Board certification or subspecialty in Geriatrics preferred.
• Current DEA certificate and BLS certification.
• Ability to work within a PACE interdisciplinary team.

Responsibilities

• Provides comprehensive medical care guided by PACE benefits.
• Leads and participates in IDT care planning.
• Reviews care plans at least every 6 months.
• Collaborates with CEI nurse practitioners and physician assistants.
• Provides after-hours telephonic medical coverage on a rotating basis.
• Coordinates care with hospitalists and manages hospital/ED utilization.
• Ensures patient medical records and care plans are accurately documented.
