Center for Elders’ Independence is seeking a Primary Care Physician to deliver comprehensive medical care for participants in the PACE program. The physician provides leadership to the interdisciplinary team to ensure integrated care planning and execution, guiding medical decisions and coordinating with nursing and ancillary staff.

The role requires an MD with California license, ongoing CME for license renewal, and experience with frail elderly populations.

Competenze

• MD with CA license required.
• CME requirements to renew license.
• Experience with frail/elderly population preferred.
• Board certification in Internal Medicine or Family Practice desired.
• Geriatrics subspecialty preferred.

Mansioni

• Provide comprehensive primary medical care as guided by the PACE benefit.
• Lead and educate the interdisciplinary team on medical issues.
• Review and sign care plans every 6 months.
• Collaborate with CEI NPs and PAs and supervise as needed.
• Coordinate diagnostics and follow-up care for participants.
• Document clinical interactions in the EMR and participate in QAPI.
