Job summary

InnovAge is seeking a Physician responsible for the medical care of enrolled geriatric participants across clinic, day center, homes and care facilities. Close coordination with hospital-based providers is essential.

The role requires an MD/DO with DEA license, board certification in primary care, and at least one year of experience with frail or elderly populations. A comprehensive benefits package and opportunities to influence patient-centered care are included.

Qualifications

• MD/DO degree and current state medical license with DEA for full prescriptive authority.
• Board certification in a primary care specialty.
• Minimum of one year working with frail or elderly populations.
• Not on CMS Preclusion List.

Responsibilities

• Provide medical care to participants, including evaluation, diagnosis, and management, including prescribing and follow-up.
• Document care in the participant's medical record and support EMR enhancements as needed.
• On a rotational basis provide after-hours on-call coverage; triaged by an RN with limited physician involvement.
• Participate in interdisciplinary teams to formulate comprehensive care plans.
• Contribute to quality improvement initiatives within InnovAge PACE programs.
• Present input to initiate and facilitate new medical programs and processes in support of strategic goals.
• Provide leadership to set the department tone, morale, teamwork, and a positive culture.
• Perform additional duties as assigned.
