WelbeHealth, LLC. is seeking Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics to deliver value-based care through our PACE program.

Qualifications

• Residency completion in ACCME-accredited program.
• Master's or Doctorate degree required.
• Board Certification preferred for eligibility.

Responsibilities

• Perform comprehensive in-person assessments with the interdisciplinary team to develop personalized care plans.
• Coordinate discharge for acute, short-term, and long-term placement.
• Limit hospital rounds to focus on comprehensive care, seeing ~8 patients per day.
