Renewal Rehab in Michigan is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role evaluates residents, develops individualized treatment plans, and delivers evidence-based interventions to improve ADLs, mobility, cognition, and safety. You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active OT license in Michigan.
• Experience with geriatric populations.
• Ability to work onsite in Michigan facilities.
• Knowledge of EMR documentation and charting.

Responsabilités

• Evaluate residents in skilled nursing and post-acute facilities.
• Develop individualized treatment plans for each resident.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety.
• Document assessment and progress in the EMR.
