Renewal Rehab in Michigan is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. You will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Formación

• Must hold an OT license in the state.
• Geriatrics experience preferred.
• Ability to assess residents and develop treatment plans.
• Ability to educate families and caregivers.

Responsabilidades

• Evaluate residents in skilled nursing and post-acute facilities.
• Develop individualized treatment plans and goals.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety.
• Document progress and clinical data in the EMR.
