Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. You will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active Occupational Therapist license and eligibility to practice in Ohio.
• Experience in skilled nursing or post-acute settings preferred.
• Ability to develop individualized treatment plans.
• Strong documentation and communication skills.

Responsibilities

• Assess residents and evaluate functional abilities (ADLs, mobility, cognition, safety).
• Develop individualized treatment plans and implement evidence-based interventions.
• Document care in EMR and maintain accurate records.
• Collaborate with an interdisciplinary team to coordinate care.
• Educate families and caregivers on treatment goals and home programs.
