Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. The onsite role focuses on evaluating residents, creating individualized treatment plans, and delivering evidence-based interventions to enhance ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold a state license as an Occupational Therapist.
• Experience with senior populations preferred.
• Onsite role in skilled nursing and post-acute settings.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
