Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualificações

• Must be a licensed OT in Illinois.
• Experience providing geriatric care in skilled nursing/post-acute settings preferred.
• Strong documentation and communication skills.

Responsabilidades
