Job summary

Renewal Rehab is actively seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. The onsite role involves evaluating residents, creating individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold a valid Occupational Therapy license.
• Experience in skilled nursing or post-acute settings preferred.
