Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role evaluates residents and delivers interventions to improve ADLs, mobility, cognition, and safety, with EMR documentation and family education.

You will collaborate with an interdisciplinary team, receive mentorship and growth opportunities, and make a daily impact on seniors’ functional outcomes and quality of life.

Qualifications

• Licensed Occupational Therapist required.
• Experience with seniors in skilled nursing or post-acute settings preferred.
• Strong documentation skills and patient-centered approach.
• Ability to work onsite and collaborate with an interdisciplinary team.

Responsibilities

• Evaluate residents and determine functional needs.
• Develop individualized treatment plans for each resident.
