Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT required.
• Experience in skilled nursing or post-acute settings preferred.
• Onsite, full-time role in Knox/Kilwaukee area serving seniors.

Responsibilities

• Evaluate residents and contribute to assessments.
• Develop individualized treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.
• Document in EMR and maintain accurate records.
• Collaborate with an interdisciplinary team and educate families and caregivers.
