Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist required.
• Experience delivering therapy to seniors in SNF/post-acute settings preferred.
• Ability to evaluate residents and develop individualized treatment plans.

Responsibilities

• Evaluate residents and determine functional goals.
• Develop personalized treatment plans for ADLs, mobility, cognition, and safety.
• Deliver evidence-based interventions.
• Document patient progress in EMR.
• Collaborate with interdisciplinary team and educate families/caregivers.
