Job summary

Provider Partner Health Plan seeks an experienced Nurse Practitioner to oversee ISNP member care in designated skilled nursing facilities. You will work with primary care physicians to create preventive plans, order tests, prescribe medications, and guide short- and long-term care goals.

In this role you will act as a clinical liaison, ensure timely, coordinated care, and maintain standard documentation while collaborating with families and the care team.

Qualifications

• Licensed Nurse Practitioner with current state licensure.
• Experience in SNF/LTC settings and case management.
• Knowledge of Medicare/Medicaid requirements.
• Proficiency with electronic health records and data reporting.
• Ability to work autonomously within regulatory standards.

Responsibilities

• Conduct comprehensive health assessments with autonomy.
• Collaborate with PCP to create preventive care plans.
• Communicate with care team to ensure seamless transitions.
• Order and interpret diagnostic tests; prescribe medications as needed.
• Serve as medical liaison to interdisciplinary staff and families.
