Job summary

Provider Partners Health Plan is seeking an Institutional Special Needs Plan (ISNP) Nurse Practitioner to deliver coordinated, comprehensive care at designated skilled nursing facilities. The NP collaborates with the member’s PCP to provide preventive care, diagnose illnesses, order tests, prescribe medications, and develop short- and long-term care plans.

The NP serves as medical liaison to the interdisciplinary team, families, and staff, ensuring seamless service delivery and adherence to PPHP

Qualifications

• Minimum 3 years clinical care experience.
• Strong knowledge of Medicare/Medicaid practices.
• Familiarity with I-SNP MOC and population.
• Proficiency with clinical documentation standards.

Responsibilities

• Performs autonomous comprehensive health assessments.
• Collaborates with PCP to create prevention-focused care plans.
• Documents per PPHP standards and Model of Care.
• Adheres to state scope of practice.
• Communicates with physicians and families during transitions of care.
• Facilitates a coordinated plan of care to avoid duplications.
