Job summary

Provider Partners Health Plan is seeking an ISNP Nurse Practitioner to deliver comprehensive care at designated skilled nursing facilities. You will collaborate with the member’s PCP to provide preventive care, diagnose acute and chronic illnesses, order tests, prescribe medications, and create short- and long-term care plans for PPHP members and their families.

Qualifications

• Clinical competence and adherence to evidence-based practice.
• Strong computer skills; able to learn and use new software and export data.

• Minimum of 3 years clinical care experience.
• Experience in skilled nursing/long-term care and case management preferred.
• Knowledge of Medicare/Medicaid and I-SNP MOC.
• Proficiency with nursing/medical databases and Microsoft programs.
• Positive, autonomous, and problem-solving mindset.

Responsibilities

• Conduct autonomous health assessments and develop care plans.
• Collaborate with PCP to create preventive, early-detection care plans.
• Document per PPHP standards and communicate across care team.
• Adhere to state scope of practice and regulations.
• Coordinate transitions of care with physicians and families.
• Provide timely and coordinated medical liaison within the facility.
