Job summary

Provider Partners Health Plan is seeking an Institutional Special Needs Plan (ISNP) Nurse Practitioner to deliver coordinated care for PPHP members in skilled nursing facilities. You will partner with the member's PCP to implement preventive care, diagnose acute and chronic conditions, order tests, prescribe medications, and create short- and long-term care goals.

This role requires independent thinking and strong collaboration with the care team.

Qualifications

• Licensed Nurse Practitioner in the appropriate state (Acute, Adult/Gerontological or Family).
• Experience in SNF/long-term care settings preferred.
• DEA/CDS licensure as required by state practice.

Responsibilities

• Perform comprehensive health assessments with autonomy.
• Collaborate with PCP to develop prevention-focused care plans.
• Adhere to documentation standards and care model guidelines.
• Work within scope of practice and coordinate transitions of care.
• Communicate with physicians, families, and care team effectively.
• Provide clinically appropriate plans to avoid unnecessary testing.
