Job summary

Provider Partners Health Plan seeks an Institutional ISNP Nurse Practitioner to deliver coordinated care for PPHP members in designated skilled nursing facilities. The NP collaborates with the member’s primary care physician to ensure seamless delivery and acts as a medical liaison to the interdisciplinary staff.

Qualifications

• Clinical competence and understanding of evidenced-based practice.
• Minimum of 3 years clinical care experience.
• SNF/Long Term Care experience preferred.
• Case management experience preferred.
• Knowledge of Medicare/Medicaid requirements.
• Proficiency with nursing/medical databases and Microsoft programs.

Responsibilities

• Demonstrates autonomy and proficiency in comprehensive health assessments.
• Collaborates with PCP to create a preventive, early-detection care plan.
• Adheres to PPHP documentation and communication standards.
• Complies with state scope of practice where licensed.
• Communicates with physicians and family during transitions of care.
• Coordinates care team to avoid unnecessary testing and fragmentation.
