Job summary

Total Life, Inc. seeks a nurse practitioner to lead longevity days in partner senior living communities across the Inland Empire. You will host on-site educational talks, then conduct one-on-one longevity visits, supported by a geriatric-focused program and CMO guidance.

You will evaluate residents for weight management, coordinate with primary care, and document all encounters to Medicare standards using our EHR system. Travel and group presentations are part of the role.

Qualifications

• Active California RN license and NP certification.
• NP Furnishing Number from California RN board.
• Current national NP board certification.
• Active DEA registration for California and CURES registration.
• Ability to practice under standardized procedures with supervising physician in CA.

Responsibilities

• Lead longevity visits (45–60 minutes) after group educational session.
• Identify residents for weight management and GLP-1 therapy when appropriate.
• Coordinate with primary care provider for follow-up care.
• Document encounters accurately to Medicare standards in the EHR.
• Represent Total Life professionally with residents, families, and staff.
• Travel to assigned senior living communities in Inland Empire area.
