Job summary

Total Life, Inc. in Phoenix, AZ seeks nurse practitioners for an in-person contract role leading longevity days at partner senior living communities. Each site day begins with an educational talk, followed by one-on-one longevity visits with residents who sign up.

You will be trained by the Chief Medical Officer and start with a site day alongside a team member. Weekday on-site days grow with bookings and require travel within the West Valley.

Qualifications

• Active, unrestricted Arizona APRN certificate with prescribing authority.
• Current national NP board certification.
• Active DEA registration on file with the State Board.
• Clinical experience caring for adults 65+.
• Experience with Medicare documentation and E/M coding.
• Malpractice insurance covering in-person care.
• Valid driver's license and reliable transportation.
• Ability to complete onboarding requirements (background check, TB, vaccinations).
• Weekday availability for on-site days in West Valley, Phoenix area.

Responsibilities

• Conduct 45–60 minute longevity visits following a geriatric assessment.
• Coordinate follow-up care with primary care providers as needed.
• Document encounters accurately to Medicare standards in the EHR.
• Lead educational talks for residents and manage sign-ups for visits.
• Travel to assigned senior living communities in the West Valley.
• Represent Total Life professionally with residents and staff.
