Job summary

Total Life in Phoenix, AZ, seeks nurse practitioners for an in-person longevity program at partner senior living communities. You will conduct comprehensive geriatric visits, review medications, assess cognition and mood, and educate residents on healthy aging.

You will collaborate with our CMO-guided team, document to Medicare standards, and travel to West Valley sites as needed. This contractor role offers flexible hours and ongoing professional development.

Qualifications

• Active, unrestricted Arizona APRN certificate with prescribing authority.
• Current national NP board certification.
• DEA registration on file with the AZ Board of Nursing and PMBP.
• Malpractice insurance coverage for in-person care.

Responsibilities

• Conduct 45–60 minute longevity visits based on a geriatric assessment, medication review, cognition, mood, sleep, nutrition, mobility and functional status.
• Evaluate eligible residents for weight management, including GLP-1 therapy when appropriate.
• Identify residents needing follow-up care and coordinate with primary care providers.
• Document every encounter to Medicare standards in our EHR.
• Represent Total Life professionally with residents, families and staff at partner communities.
• Travel to assigned senior living communities in the West Valley (Phoenix) area.
