Job summary

• Active FNP/AGNP/AHACNP or PA license.
• 3 years of geriatric clinical nursing or rehab experience.
• Electronic Health Records experience and MS Office proficiency.

Responsibilities

• Assess patients' medical and mental health needs and develop a plan of care.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order lab work, diagnostics, and consultations.
• Collaborate with RN case managers as part of an integrated care team.
