Job summary

Ultra Health Providers seeks a Nurse Practitioner to deliver comprehensive medical care in skilled nursing and senior living settings. You will practice autonomously, manage complex conditions, and build trusted clinical relationships with residents, supported by a collaborative interdisciplinary team.

The role emphasizes independence, timely responses to urgent needs, proactive care, and adhering to regulatory and payer standards within HIPAA guidelines.

Qualifications

• Active, unrestricted NP license in the applicable state.
• Current national NP certification (ANCC/AANP or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.
• Eligibility to work in the United States.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic conditions.
• Perform admissions and follow-up assessments, including exams and medication review.
• Monitor patient changes and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe within scope of practice.
• Collaborate with facility leadership and interdisciplinary teams.
• Communicate clearly with residents and families about diagnoses and plans.
• Maintain accurate documentation in the EMR per standards.
• Participate in quality and care coordination efforts.
