Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You will work with a team of providers and case managers to deliver coordinated, high-quality care.

Expect a competitive benefits package and opportunities for career growth. APRN or PA license is required, with an accredited APRN/PA program degree and 3 years of geriatric clinical experience.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
