Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with primary care or internal medicine experience, preferably in geriatrics, to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers on personalized, coordinated care.

Our APRN/PA providers work mainly in nursing homes or assisted living communities enrolled in Medicare Advantage plans, delivering care that reduces hospital visits and enhances quality of life.

Qualifications

• License as FNP/AGNP/AHACNP or PA is required.
• Degree from accredited APRN or PA program.
• 3 years of clinical nursing or rehab experience in geriatric populations.
• EHR experience and basic MS Office skills.

Responsibilities

• Assess patients’ medical and mental health needs with a proactive care plan.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, procedures, and consultations.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
