Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric/long-term care experience to join our team. You will deliver patient-centered primary care within nursing homes and assisted living settings, collaborating with physicians and case managers to improve outcomes and reduce hospital visits.

You will hold an APRN or PA license, have 3 years geriatric clinical experience, and work in a supportive environment with competitive pay, benefits, and professional growth

Qualifications

  • Masters degree or higher in Nursing or related field.
  • APRN or PA license required.
  • 3 years’ experience in clinical nursing or rehab in geriatric populations.
  • Electronic Health Records experience.
  • Proficiency in Microsoft Word, Outlook, and Excel.

Responsibilities

  • Provide advanced practice care to long-term care residents.
  • Prescribe medications and order labs and referrals.
  • Collaborate with RNs case managers and supervising physician.
  • Ensure compliance with regulations and care plans.
