Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics focus to join our care team in long-term care settings. You will deliver personalized, coordinated health care to residents enrolled in Medicare Advantage plans, collaborating with RNs, case managers, and physicians to optimize outcomes and reduce hospital visits.

You must hold an APRN or PA license (FNP/AGNP/AHACNP or PA) and have at least 3 years of clinical experience with older adults.

Qualifications

• Experienced APRN/PA with geriatrics focus.
• License as FNP/AGNP/AHACNP or PA required.
• EHR experience and MS Office proficiency.

Responsibilities

• Assess patients and create proactive care plans.
• Ensure compliance with local, state, and federal rules.
• Prescribe medications and order tests as needed.
• Monitor treatment responses and adjust plans.
• Collaborate with RN case managers and care team.
