Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with geriatric experience. You will work in primary care or internal medicine within long-term care settings, coordinating care with a team of providers and case managers to improve outcomes for residents enrolled in Medicare Advantage plans.

Qualifications

• Must hold an active APRN or PA license.
• Masters degree or higher in nursing or related field.
• 3 years’ clinical nursing or rehab experience in geriatric populations.
• Experience with Electronic Health Records and Microsoft Office applications.

Responsibilities

• Assess medical and mental health needs and develop proactive care plans.
• Ensure compliance with local, state, and federal regulations relating to clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor treatment response and modify plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
