Job summary

TruHealth seeks an advanced practice provider (NP or PA) with geriatrics experience to join a team delivering proactive, coordinated care in nursing homes and assisted living communities enrolled in Medicare Advantage plans.

You will exercise independent judgment, consult with a supervising physician as needed, and collaborate with facility staff and health plan leadership to implement TruHealth’s national care model while complying with regulations.

Qualifications

• APRN/PA licensure required with credentials in nursing or medicine.
• Master's degree or higher from an accredited APRN or PA program.
• Minimum 3 years’ experience in geriatric nursing or rehab settings.
• Experience with Electronic Health Records (EHR) systems.
• Proficiency in Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create a proactive plan of care.
• Ensure compliance with local, state, and federal regulations governing clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
