Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with a geriatrics focus to improve outcomes for long‑term care residents in nursing homes or assisted living communities enrolled in Medicare Advantage plans.

You will exercise independent judgment, coordinate with supervising physicians, and work with facility partners to implement TruHealth’s national model of care while ensuring compliance with applicable regulations.

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, procedures and consultations.
• Monitor treatment responses and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
