Job summary

TruHealth is seeking an advanced practice provider, either an NP/PA, with experience in primary care or internal medicine and a geriatrics focus. This role involves working primarily in nursing homes or assisted living communities to coordinate care for residents enrolled in Medicare Advantage plans.

• Masters degree or higher from an accredited program.
• APRN/NP or PA license required.
• 3 years’ experience in geriatric nursing or rehab.
• Electronic Health Records experience.
• Proficient with Microsoft Word, Outlook, and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create care plans.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and consultations.
• Monitor treatment plans and adjust as needed.
• Collaborate with RN case managers within an integrated care team.
