Job summary

TruHealth seeks an advanced practice provider (NP or PA) with geriatrics/primary care experience to improve outcomes for residents in long-term care facilities. You will work within nursing homes or assisted living communities enrolled in Medicare Advantage plans, delivering coordinated health care to improve quality of life.

Qualifications

• APRN/PA license and primary care or geriatrics experience required.
• 3 years’ clinical nursing or rehab experience in geriatrics preferred.
• Experience with Electronic Health Records (EHR).
• Proficiency with Microsoft applications (Word/Outlook/Excel).

Responsibilities

• Assess patients’ medical and mental health needs and create plans of care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment response and adjust care plans as needed.
• Collaborate with RN case managers and the care team.
