Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for residents in long-term care. You will collaborate with a team of clinicians and case managers to deliver personalized, coordinated care in nursing homes or assisted living communities.

You will work under supervision as appropriate, following federal and state regulations while implementing our national model of care.

Qualifications

• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.
• FNP, AGNP, AHACNP or PA license required.

Responsibilities

• Assess patients’ medical and mental health needs and provide a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers as part of the care team.
