Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to improve outcomes for residents in long-term care. You will work with a team of providers and case managers to deliver personalized, coordinated healthcare within nursing homes or assisted living facilities enrolled in Medicare Advantage plans.

You will exercise independent judgment for acute and chronic conditions, consulting a supervising physician as needed, and help implement TruHealth's national model

Qualifications

• Masters degree or higher in nursing or related field.
• APRN/NP or PA license required.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Microsoft Word/Outlook/Excel proficiency.

Responsibilities

• Assess patients’ medical and mental health needs and create a proactive care plan.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications, order labs, and coordinate consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers and the care team within a facility.
