Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably with geriatrics focus. This position gives you the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers.

Qualifications

• Masters or higher degree required for APRN/PA practice.
• Degree from an accredited APRN or PA program.
• APRN/PA license required and active.
• 3 years’ experience in geriatrics or rehab settings.
• Experience with EHR systems and MS Office.

Responsibilities

• Assess medical and mental health needs with proactive care plans.
• Ensure compliance with local, state and federal regulations.
• Prescribe medications and order labs, tests and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated team.
