Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to work in nursing homes or assisted living communities. You will deliver personalized, coordinated care and collaborate with a team to improve resident outcomes and reduce rehospitalizations.

A strong background in geriatric primary care and EHR experience is essential; competitive pay and career growth await.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a care plan addressing conditions and barriers to care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patient progress and adjust treatment plans as needed.
