Job summary

TruHealth is seeking an advanced practice provider (NP/PA) to serve residents in long-term care settings, primarily within nursing homes or assisted living facilities enrolled in Medicare Advantage plans. You will evaluate medical and mental health needs, coordinate care plans, and work with a multidisciplinary team to improve resident outcomes.

Qualifications

• APRN or PA license required.
• Experience in geriatric populations preferred.
• Ability to implement coordinated care in a multi-disciplinary team.

Responsibilities

• Assess patients’ medical and mental health needs and develop a plan of care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and diagnostics.
• Coordinate with RN case managers and health plan leadership.
