Job summary

TruHealth, part of American Health Partners, is seeking an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You will work within nursing homes or assisted living communities, collaborating with a team to provide personalized, coordinated health care that improves quality of life and reduces hospital visits.

You will exercise independent judgement to treat acute and chronic conditions, consulting with a supervising physician as needed

Qualifications

• Masters or better.
• APRN/PA license required.
• 3 years of clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Microsoft Word/Outlook/Excel proficiency.

Responsibilities

• Assess patients' medical and mental health needs and develop a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers and health plan leadership.
