Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics focus to improve care for seniors in long-term care facilities. You will deliver personalized care within a coordinated team and work closely with case managers and supervising physicians to implement our national model of care.

The role requires independent judgment, strong knowledge of geriatric health issues, and adherence to regulations. Excellent benefits and growth opportunities are included.

Qualifications

• APRN/NP or PA license required.
• 3 years’ geriatric clinical nursing or rehab experience.
• EHR experience and MS Office proficiency.

Responsibilities

• Assess medical and mental health needs; develop plan of care.
• Prescribe medications; order labs, diagnostic procedures and consultations.
• Coordinate with RN case managers and facility staff.
• Ensure compliance with local, state, and federal regulations.
