Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to join our team serving seniors in long-term care facilities. You will work with physicians and case managers to deliver coordinated, high-quality care in nursing homes or assisted living communities enrolled in Medicare Advantage plans.

The role emphasizes independent clinical judgment, adherence to regulations, and collaboration with facility partners.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.

Responsibilities

• Assess patients’ medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal agency requirements.
• Prescribe medications and order lab work, diagnostics and consultations.
• Monitor patients’ progress and modify treatment plans.
• Collaborate with RN case managers as part of an integrated care team.
