Job summary

TruHealth is recruiting an advanced practice provider (NP or PA) with geriatric experience to deliver coordinated primary care in nursing homes and assisted living settings. You will work with a multidisciplinary team to improve outcomes for long-term care residents under our national care model, guided by federal and state regulations.

The role emphasizes independent judgment in treating acute and chronic conditions, with supervising physician support as needed, and opportunities for career

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years' experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• MS Word/Outlook/Excel knowledge.

Responsibilities

• Assess patients' medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, tests and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers and health-plan leadership.
