Total Life, Inc. is seeking nurse practitioners for an in-person contract role to lead longevity days at partner senior living communities in the Chicago suburbs. You will present to residents, then conduct 45–60 minute longevity visits based on a comprehensive geriatric assessment.

Trained by our CMO, you will work alongside a team to ensure Medicare-compliant documentation and high-quality care. Site days are weekday-based with hours expanding as bookings grow.

Qualifications

• Active Illinois APRN license and Illinois RN license.
• Current national NP board certification.
• Active DEA registration and Illinois mid-level practitioner license.
• Ability to practice under a physician collaborative agreement unless APRN-FPA.
• Clinical experience caring for adults 65+.
• Experience with Medicare documentation and E/M coding.

Responsabilités

• Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment.
• Evaluate eligible residents for weight management, including GLP-1 therapy where appropriate.
• Identify residents needing follow-up care and coordinate with their primary care provider.
• Document encounters in the EHR to Medicare standards.
• Represent Total Life professionally with residents, families and staff.
• Travel to assigned senior living communities in the Chicago suburbs (Joliet-Palatine).
