Job summary

Total Life is seeking a Nurse Practitioner to lead longevity days at partner senior living communities in the Auburn, AL–Columbus, GA area. You will give 45-60 minute longevity visits after a geriatric assessment and provide education to residents.

Trained by our CMO, you will document all encounters to Medicare standards, coordinate with primary care, and travel regularly for on-site days. A strong background in Medicare documentation and adult 65+ care is required.

Qualifications

• Active, unrestricted Alabama NP license
• Active Georgia APRN authorization to see Columbus residents
• Current national NP board certification
• DEA registration for AL and GA
• Ability to practice under a physician collaborative agreement (Alabama)
• Ability to practice under a nurse protocol agreement (Georgia)
• Experience caring for adults 65+ and Medicare documentation

Responsibilities

• Conduct 45-60 minute longevity visits following geriatric assessment
• Assess weight management eligibility and GLP-1 therapy where appropriate
• Coordinate follow-up care with primary care providers
• Document encounters per Medicare standards in the EHR
• Represent Total Life professionally with residents and staff
• Travel to assigned senior living communities in Auburn, AL–Columbus, GA area
