Total Life, Inc. is seeking nurse practitioners for in-person longevity days across senior living communities in the Capital Region (Albany-Schenectady-Troy). You will run education talks, then conduct 45–60 minute longevity visits with comprehensive geriatric assessment.

The role requires NY RN and NP certifications, DEA registration, and 3 hours biennial prescriber education. Travel is required; on-site weekdays with variable hours.

Qualifications

• Active, unrestricted New York RN license and NP certification.
• Current national NP board certification.
• Active DEA registration and NY EPCS setup.

Responsabilités

• Lead longevity visits after a short resident education session.
• Coordinate with primary care providers and specialists as needed.
• Document encounters to Medicare standards in the EHR.
• Travel to assigned senior living communities in Capital Region.
• Represent Total Life professionally with residents and staff.
