Job summary

Total Life is seeking nurse practitioners to lead longevity days at partner senior living communities in the Inland Empire. You will present to residents, then perform 45–60 minute geriatrics visits with assessments, med reviews, and cognitive screening, supported by the CMO.

Site days are weekdays with variable hours and travel between communities. You will be trained in geriatric assessment tools and Medicare documentation, with a flexible contractor schedule that grows as bookings increase.

Qualifications

• Active, unrestricted California RN license required.
• Certified Nurse Practitioner with California Board of RN, NP Furnishing Number.
• Current national NP board certification and DEA registration in CA.
• Ability to practice under standardized procedures with physician supervision.
• Experience with Medicare documentation and E/M coding.
• Comfort presenting to groups of older adults and families.

Responsibilities

• Lead longevity days at partner senior living communities, including an educational talk and 1-on-1 visits.
• Conduct 45–60 minute geriatric assessments, medication review, and cognitive screening.
• Coordinate follow-up care with PCPs and specialists as needed.
• Document encounters in Medicare-compliant EHR per standards.
• Travel to Inland Empire across assigned senior living communities.
• Represent Total Life professionally to residents, families, and staff.
